RATIFICATION OF REASSIGNMENT OF STUDENTS TO SCHOOLS

8. RATIFICATION OF REASSIGNMENT OF STUDENTS TO SCHOOLS
Description

Reassignment of students to schools within the county have been approved by the Reassignment Review Committee in accordance with policies with the School Board.  They are presented for formal ratification.

AGREEMENTS/CONTRACTS

9. AGREEMENT BETWEEN INVO HEALTHCARE ASSOCIATES AND THE SCHOOL BOARD OF SARASOTA COUNTY, FLORIDA FOR PHYSICAL/OCCUPATIONAL/SPEECH THERAPY SERVICES
Description

Physical, occupational and speech therapy are related services required by both Federal and State law for students with disabilities.  Contracts with agencies such as Invo Healthcare Associates, assist in providing these necessary services, when direct hire staff are not available.



Recommendation
That the agreement between Invo Healthcare Associates and the School Board of Sarasota County, Florida, be approved as presented.

10. AGREEMENT BETWEEN ACCOUNTABLE HEALTHCARE STAFFING, INC. AND THE SCHOOL BOARD OF SARASOTA COUNTY, FLORIDA,
Description

FIELD TRIPS

11. APPROVAL FOR VENICE HIGH VARSITY FOOTBALL TEAM TO TRAVEL TO CAMDEN COUNTY, GEORGIA TO PLAY SECOND GAME IN TWO YEAR CONTRACT AT CAMDEN COUNTY HIGH SCHOOL ON AUGUST 24-25, 2017.
Description
This field trip is for Venice High Varsity Football team to play the second year in a two year contract.  This allows players to participate in 10 games against a comparable team according to division standards. Camden County High School came to Venice High School to play the first year.

Recommendation
That the field trip for Venice High Varsity Football team to travel to Camden County, Georgia and play the second game of a two year contract at Camden County High School on August 24-25, 2017 be approved as presented. 

CONSTRUCTION

12. ACCEPTANCE OF CONSTRUCTION SERVICES' PROJECT COMPLETION AND APPROVAL OF FINAL PAYMENT TO CONTRACTOR FOR THE ASHTON ELEMENTARY PORTABLE DEMO AND RELOCATION PROJOECT
Description
All work has been completed and all required close-out documents have been received from the contractor. The final project costs have been internally audited and are enclosed herein in Change Order No.1 for approval. The Director of Construction Services has determined that the project is complete in accordance with state statues.

Recommendation
That the project be accepted as complete and the final payment to the contractor be approved as presented.

13. APPROVAL OF AMENDMENT #6 TO ARCHITECT'S CONTRACT FOR THE SUNCOAST TECHNICAL COLLEGE SOUTH CAMPUS
Description
The scope of work for this amendment is to provide additional design services for the modifications to the documents requested by the City of North Port Fire Department, re-design of Building #02 Industrial Labs and modify the equipment and piping to change from LP gas to Natural Gas. The total amount for these changes shall not exceed $21,410.00. These funds are available through the project budget.

Recommendation
That Amendment #6 to the Agreement dated February 21, 2017, with Schenkel & Shultz, Inc., for the Suncoast Technical College South Campus be approved as presented.

14. ACCEPTANCE OF CONSTRUCTION SERVICES' PROJECT COMPLETION AND APPROVAL OF FINAL PAYMENT TO CONTRACTOR FOR THE WILKINSON ELEMENTARY PLAYGROUND RESTROOM BUILDING
Description
All work has been completed and all required close-out documents have been received from the contractor. The final contract costs have been internally audited and are enclosed herein in Change Order #1 for approval. The Director of Construction Services has determined that the project is complete in accordance with state statutes.

Recommendation
That the Wilkinson Elementary Playground Restroom Building Construction Project be accepted as complete and the final payment to the contractor be approved as presented.

15. APPROVAL AND/OR RATIFICATION OF CONSTRUCTION SERVICES' CHANGE ORDERS
Description
The Construction Services' Change Orders are summarized on the enclosure for ease of review. These change orders collectively total a decrease of ($435,395.55) in gross contract dollars. Included in these change orders are ($405,862.23) credit for Direct Material Purchase; the corresponding Direct Material Purchase Orders of ($382,673.80) result in a net of $23,188.43 in sales tax savings.

Recommendation
That the Construction Services' Change Orders be approved and/or ratified as presented.
